Certified Diabetic Education Programs
Diabetes Self-Management Training Program (DSMT)

If you have diabetes or at risk for diabetes, MODE offers you resources to teach you how to maintain a healthy lifestyle. One such program is called the DIabetes Self-Management Training Program or DSMT. This program is recognized by the American Diabetes Association and is facilitated by a Certified Diabetes Educator.

 
What is a Certified Diabetes Educator (CDE)?
A Certified Diabetes Educator (CDE) receives periodic certification from the National Certification Board for Diabetes Educators and is recognized as the most qualified health care professional to provide DSMT. CDE's are required to have a licensure or registration as a health care professional or minimum of a master’s degree in various, specified healthcare fields. In addition, a CDE must have a minimum of two years of professional practice experience in diabetes self-management training in a health care setting and at least 1,000 hours of diabetes self-management training experience within the last five years. All CDE's are employed in a primary role as a diabetes educator a minimum of four hours per week.
 
What will I learn in the program?

You will take part in an interactive process to assess your needs and identify specific diabetes preventative or self-management goals. This is done by describing the diabetes disease process and treatment options to you in a manageable, easy to understand way. When you have completed the program you will learn...

  • Appropriate nutritional management
  • How to add physical activity into your lifestyle
  • Utilize medications for effectiveness (if applicable)
  • Monitor blood glucose and urine ketones (when appropriate) and use the results in an effective way.
  • Prevent, detect, and treat acute and chronic complications
  • Set goals to promote health and problem-solving for daily living
  • Promote preconception care, management during pregnancy, and gestational management.
 
How do find a DSMT Program in my area?
There are locations across the State of Michigan that provide the DSMT Program in a variety of settings. Some facilities offer training in groups and/or one-on-one sessions.
