Job Location: Downriver area/ local travel to PCP offices

Job Description: Medical Network One/ Michigan Institute for Health Enhancement offers an innovative chronic disease model for delivery of self-management education.
This position works as part of a Travel Team of nurses, dietitians, mental health specialists and more, to help patients learn disease management skills and behavior change tools directly in their primary care physician’s office.
